Job description

Erect a modular (Kwikstage-type) scaffold to a maximum working platform height of 4 m along the eastern elevation of the dwelling: three bays including one return, two lifts with internal ladder access, full guardrails, mid-rails, toeboards and edge protection. Scaffold is for bricklaying — duty rating: heavy duty, 675 kg per platform per bay. Do not exceed rated load.

High risk construction work

This task involves a risk of a person falling more than 2 metres, so a SWMS is required under the Work Health and Safety Regulation 2017 (NSW). This SWMS must be read with: AS/NZS 1576 (Scaffolding), AS/NZS 4576 (Guidelines for scaffolding), the manufacturer's erection instructions, and the Managing the Risk of Falls at Workplaces Code of Practice.

Before work starts (mandatory): All crew must attend the toolbox talk, review this SWMS, confirm the task scope and any site changes with the Site Supervisor (Priya Nair) and the Principal Contractor's Site Manager, and sign the sign-on register below. Work must not start until the task has been confirmed with these personnel.

Hazard not listed in this SWMS? STOP WORK immediately. Report the hazard to the Site Supervisor (Priya Nair). Work must not resume until the SWMS has been reviewed and revised, controls are in place, and the change has been communicated to all crew at a revised toolbox talk.
