Why this form exists: Everyone on site is responsible for spotting and reporting environmental hazards and resource efficiency issues. Use this form for anything that could cause harm to the surrounding environment (for example, dust or noise leaving the site, or contaminated waste in the wrong bin) or anything that uses more resources than the job needs (for example, lights and machinery left running when nobody is using them, or materials over-ordered and thrown away). Hand the completed form to your direct supervisor on the same day. Your supervisor decides whether further escalation is needed.

Your role today

You are a first-year apprentice with Harbourline Constructions. Your direct supervisor is Priya Sharma, Site Supervisor. The leading hand is Jack Torrens. Read the two situations below and work out how each report form should be filled in.

Situation A

Tuesday 14 May, about 7:15 am. Walking past the diesel generator on the eastern site boundary, you and your workmate Daniel Ahmadi notice diesel dripping from a loose fuel line fitting. A pool of fuel roughly half a metre wide has formed, and after overnight rain it is trickling towards the stormwater pit about two metres away. You fetch the spill kit, place absorbent socks around the pool, put the drain cover mat over the stormwater pit, and slide a drip tray under the fitting. The leak has slowed but the fitting still needs repair.

Situation B

Thursday 16 May, about 12:40 pm. At the wash-down bay next to the site sheds, you find a hose running unattended at full flow. Nobody is around. Judging by the water pooling across the bay, it has been running for at least 40 minutes. The hose has no trigger nozzle, so it cannot shut off on its own. You turn off the tap and coil the hose. This is the second time this week you have seen the hose left running.
